NOTICE:  Effective January 1, 2021 our practice will no longer be an in-network provider for any private insurance.  We are transitioning to a direct primary care patient model, and encourage you to read this page to determine if this model is right for your family.

Non-covered Benefits Fee

We understand that health insurance is a big expense for most families.  Most physicians offering our level of care, service, and physician access are “concierge” doctors that typically charge $1500 or more per year plus added charges for each visit and do not bill insurance.  We are not a concierge medical practice.  We want our high-level of accessibility and care to be affordable for the average family, and understand your desire to have your insurance billed directly for the services we provided.

For benefits not covered by insurance, however, our office charges a fee of $10 per month for an individual or $15 for a family (immediate relatives or partners living in the same household and their dependents).  If you choose to pay on an annual basis, the fee is reduced to $100 for individuals and $150 for families (prorated to November 1st) .

This fee covers the added services provided through our practice including direct on-call access to your doctor for urgent care, on-line appointment scheduling, and secure on-line communication with your doctor for non-urgent issues.  These benefits are not covered under health insurance policies and cannot be covered or reimbursed by insurers.  However, you may be able to use a Medical Flexible Spending Account or Health Savings Account funds to pay this fee.

Financial Policies

Payment for services are expected at the time of service.  With accurate insurance policy information, we will be able to access your deductible information and will expect payment for any co-pays or deductibles at the time of service.  If policy information cannot be confirmed, you will be expected to pay in full at the time of service.  If you do not have insurance and pay in full at the time of service, a prompt pay discount may be applied.

Any account balances can be access through the patient portal and can be payed by credit card on-line or by check via mail.  In the event that you fail to return payment within 30 days, a $15 service charge will be applied.  Balances left unpaid and without further arrangements within 90 days may result in dismissal from the practice, balance sent to collections, and/or notification to your insurance plan.  Any returned checks will be charged a $25 fee in addition to any bank fees.

Missed Appointments

We work extremely hard to keep scheduled appointments on-time and to provide sufficient visit time for all our patients.  We also make a considerable effort to be available for any same day visits that are needed.  To keep our appointments on-time, we respectfully ask that scheduled appointments be cancelled no less than 24 hours prior to the scheduled appointment.  Any appointments that are missed without notice or cancelled with less than 24 hours notice will incur a $50 fee.  Any patient missing three or more appointments without 24 hours notice may be asked to leave the practice.
